Parent Consent with instructions

Please read form carefully, fill out, and sign the paper in one of two ways:

1.       Digitally signing and emailing to (easy, detailed instructions below)

2.       Printing, signing, and mailing to: 20207 NW 113th Way, Alachua, FL, 32615.


Easy Steps to Digitally Sign and Email Parent Consent Form:

1.       Download the parent consent form and Save it in a known location on your computer

2.       Go to:

3.       Type in your email address  where it asks for “Email” in the center of the screen, uncheck the “keep me updated” check box, then click “Get started

4.       Click “skip” at the bottom of the page

5.       Set a password

6.       Create a signature

a.       Click on “Create Now” next to “Create a Signature”

b.      Click “Draw it in” and draw a signature by pressing down on your mouse and moving it. Then click “Next”, then “Save”. (There are other options for creating a signature that you can use instead if you find it easier)

c.       Follow these steps for creating a signature for the Participant, the Mother, and Father.

d.      Exit the signature-creating window

7.       At the top left corner of the page, click on “Sign and send

8.       Click on “UPLOAD FILES” and click on the Parent Consent 2013 PDF where you saved the document in the first step. The document should download onto the webpage.

9.       Click “Edit & Sign

10.   With your mouse, you can click anywhere and type in letters, please fill out the required information on the provided lines such as “Participant’s Full Legal Name” and “Date”.

11.   For inserting a signature, at the top left of the window is “Signature or Initials”, click it.

12.   Click “Insert signature

13.   The signature will appear on the document on the top left or somewhere random, drag it with your mouse where you would like to place it. Minimize/expand it to fit the signature lines by clicking and dragging the bottom right corner of the box surrounding the signature.

14.   After filling out all of the required information, click “Save” at the top right of the page.

15.   Type in where it says “Enter an email address” and click “Send it now

16.   Finally, go to your email address and click on the email from Hello Sign, on the email, click “Verify your email address”. The signed paper is sent to our email address. Hare Krishna.
